In this critical role, the Hospice Medical Social Worker will offer counseling, emotional support, and practical assistance to ensure that patients facing terminal illnesses can maintain their dignity and quality of life. This position requires a deep understanding of the medical and emotional needs of patients and their families, as well as the ability to navigate complex healthcare systems. Our ideal candidate will work closely with an interdisciplinary team, including physicians, nurses, and other healthcare professionals, to create and implement individualized care plans tailored to the unique needs of each patient. The Hospice Medical Social Worker will also facilitate communication between family members and healthcare providers, helping to mediate conflicts and address any concerns.
